Proximal Humerus fracture
These fractures come in several different patterns. Those treated with ORIF (plate and screw fixation) will be displaced 2 part, 3 part, or 4 part fractures in younger patients.
1-14 days: Sling at all times. Ice three times daily, 20 minutes at a time. Sponge bath only, keep dressing intact. ROM hand, wrist and elbow several times daily
2-6 weeks: Sling when up and about. May remove sling when at rest and for sleep if a sound sleeper. May shower normally. Continue to ice. May do pendulum and table slides if directed.
5-10 weeks: May discontinue sling. May begin driving. May take NSAIDs if necessary. May begin activities of daily living (self care, feeding, etc.). Initiate directed PT for active/active assist ROM
3-5 months: Continue ROM PT and advance rotator cuff strengthening. Improve adl's. May initiate light weights
6 months +: Return to activities as tolerated. Continue cuff and periscapular strength.
