Ulnar Collateral Ligament/ Tommy John Reconstruction
This is almost universally a pitcher's injury, but javalin throwers are the next best represented group.
1-10 days: The elbow is splinted in 90 degrees of flexion. Ice frequently and avoid NSAIDs and nicotine products. Keep dressing clean and dry.
2-6 weeks: Hinged elbow brace 30-90 degrees, increase 10 degrees weekly in flexion and extension. Wrist and shoulder ROM exercise. At 4 weeks begin rotator cuff strengthening exercise.
6-10 weeks: Discontinue hinged elbow brace . Initiate light throwing program with well trained therapist.
10-16 weeks: Progress throwing program. At 12 weeks, start golf, swimming and a hitting program.
16-22 weeks: Initiate interval throwing program phase I (long toss).
22-30 weeks: Advance interval throwing program to phase II.
8 months+: Advance to competitive throwing.
